Marking your ballot

When marking your ballot, make sure you are:

• Using either a blue or black ballpoint pen, OR a lead pencil
• Filling in the bubble completely (do not use check marks or “X’s”)

Returning your ballot

Once you have completed your ballot:

1. Place your ballot in the light gray, #1 Ballot Security Envelope and seal it.

   Do NOT include your voter instructions.

2. Place the now sealed, light gray, #1 Ballot Security Envelope INSIDE the dark gray, #2 certificate envelope.

3. Fill out your certificate envelope in the presence of a witness.

   • The witness MUST be 18 years or older.
   • No person, except an immediate family member of the voter, shall witness more than one voter certificate.

4. Sign your name and print your mother’s maiden name in the appropriate fields.

   If your mother’s maiden name is unknown, write “Unknown.” Do NOT leave this field blank.

5. Your witness must sign their name in the box labeled “Witness’s Signature” and print their name and address below their signature.

6. Place your completed dark gray, #2 certificate envelope INSIDE the purple, #3 return envelope. This envelope is already addressed to your parish Registrar of Voters with your return address. Please make sure you seal the return envelope.

7. Place TWO (2) stamps of postage on the return envelope and drop it in the mail at a USPS mailbox or drop box.

   You may also return your ballot in person by delivering it to your parish Registrar of Voters office.
